Travel consent form

When a minor child travels either alone or is not accompanied by her/his own parents, a document called a travel consent form must be prepared.  It is the legal authorization by the parents to such travel and must be in the possession of the traveling child or the adults accompanying him/her at all times.

Notaries at Morris & Dupuis can prepare a concise and clear form that complies with applicable legal requirements.
